SystemiQ has supported several energy companies in supporting the effective integration of human factors in safety investigations, through the review and update of investigation procedures and training, and through bespoke training courses to build internal investigator capability.  We have also participated as an integrated part of the investigation team to provide human factors specialist advice for investigations.  We have supported systemic investigations focused on a single significant incident, and by looking across a number of incidents or errors to identify trends and uncover underlying systemic issues.

We also specialise in conducting systemic reviews to help solve organisational issues relating to human error or human-system integration.  For example, we supported a fuel company in understanding human errors occurring in the application of their emergency response processes.  We engaged with operators to understand the problem, and developed recommendations to improve performance through procedure redesign, additional job aids/memory aids, and tailored high-performance skills training and scenario-based training to support capability among operational teams.
